Refund Policy

The fee amount paid for a medical program purchased by patient from American Medical General, Inc. less fees and allocated administrative costs incurred to-date, shall be refunded to the patient if the assigned patient NMC treating physician declines to prescribe the treatment program sought by the patient after the physician's receipt and evaluation of the patient's blood test report prepared by an NMC medical laboratory, NMC treating physician's evaluation of the results of the patient's current physical examination with regard to the sought treatment and NMC treating physician's assessment of the patient's medical history in relation to the patient's medical complaint. The NMC treating physician reserves the right to determine if the treatment sought is safe and appropriate for the medical condition of the patient. Only the NMC treating physician can initiate a refund of any portion of a patient's medical treatment purchase amount in accordance with this policy by declining to prescribe treatment sought by patient.

Cancellation Policy

A minimum order or enrollment in a medical treatment cancellation fee in the amount of $1200.00 shall be charged all patients with regard to any medical treatment purchased from American Medical General, Inc. cancelled for any reason by the patient subsequent to the date and time payment purchased such treatment without regard to services rendered to patient by AMG The total cancellation fee payable by patient shall exceed this minimum amount if administrative or other services have been rendered to patient by American Medical General, Inc., (AMG) staff personnel, National Medical Clinic, Inc. (NMC) or AMG physician or staff personnel, any medical laboratory or dispensing pharmacy at the request of AMG or NMC staff personnel since the date and time of patient's purchase, including, but not limited to, the following: taking of patient's medical history; establishing and maintaining patient's digital medical file; maintaining an event log of services rendered on behalf or patient; creating and organizing patient's paper medical file; sending of lab test prescription or lab test requisition to patient for patient to undergo a lab test; obtaining patient's lab report from medical laboratory; delivery of patient's lab report to treating physician; evaluation of lab report by treating physician; scheduling patient's physical examination; referring patient to a treating physician; coordinating exam appointment with patient and examining physician; obtaining patient's physical exam report; requesting prescribed treatment sought by patient; securing assessment of patient and prescribed treatment for patient from treating physician; obtaining written prescriptions; independent pharmacy dispensing and shipment of prescribed medications to patient; delivery of treatment instructions to patient; and any communications between AMG staff and patient by phone, e-mail or U.S. If patient fails to provide e-mail notice of a cancellation of a physical exam appointment to an AMG or NMC employee on a non-holiday weekday between the hours of 9:00 A.M. and 5:00 P.M. a minimum of 48 hours before an AMG of NMC scheduled appointment for patient only by sending such physical exam appointment cancellation to email address international_health@earthlink.net, then an additional $300.00 shall be added to the patient's cancellation fee as provided by this policy for failing to show for a scheduled physical exam appointment. No portion of the purchase funds shall be refunded to patient after the pharmacy has dispensed the prescribed medication purchased to the patient. Patient unconditionally and irrevocably agrees to the above stated 'Refund and Cancellation' policies posted for patients on website www.nationalmedicalclinic.com by making a credit card or other form of payment to American Medical General for a medical treatment.
